Sports Participation Forms
Please read over all the forms listed below. Then sign and complete the Athletic Acknowledgement/Consent Checklist form and return it to the Athletic Director.
Please note that you MAY NOT participate in any sport unless you have submitted a current physical examination form each school year, signed by your physician, clearing you to play.
